Immunizations & Physical Exams

 

Physical Exam

To comply with the education laws of New York State, every student in Pre-K or Kindergarten, grades 1, 3, 5, 7, 9 & 11 have a physical examination. Ideally, the child's primary health care provider should perform the physical examination. The physical must be returned to the school no later than two weeks after your child begins school. It should be understood that if the medical form is not returned, your child will be scheduled for a school physical. New York State Health Examination Form - to be completed in its entirety by private health care provider or school medical director. 

New York State law (Chapter 281) permits schools to request an oral health assessment in the following grades: school entry, Pre-K or K, 1, 3, 5, 7, 9, and 11. Your child may have a dental check-up during this school year to assess his/her fitness to attend school. Please complete Section 1 and take the form to your registered dentist or registered dental hygienist for an assessment. If your child had a dental check-up before he/she started the school, ask your dentist/dental hygienist to fill out Section 2. Dental Health Certificate Form - return the completed form to the school's medical director or school nurse as soon as possible.

ALL IMMUNIZATION DATES ARE MANDATORY AND MUST HAVE A DOCTOR'S SIGNATURE/STAMP. “Refusal to Vaccinate” is not a medical exemption. DOH and NYSED do not allow Religious Exemptions and Philosophical views. In order to comply with Section 2164 of the Public Health Law, each child must have the following immunizations (evidence of these must be provided by parent or guardian upon child's entrance):

  1. Diphtheria or Triple Vaccine (DTP, DTaP) (3-5 doses) 
  2. All children 11 years of age or older and entering grades 6 through 12 are required to have 1 dose of Tetanus, Diphtheria,            and Acellular Pertussis (Tdap) vaccine. 
  3. Polio Vaccine Trivalent (TOPV and/or IPV) 3-4 doses 
  4. Measles/Mumps/Rubella (MMR) Live Vaccine (2 doses) or positive serology Titres from HealthCare Provider 
  5. Hepatitis B (HBV) (3 doses or 2 adult doses Hepatitis B at least 4 months apart between 11 through 15 years old) 
  6. Varicella (Chicken Pox) Vaccine (2 doses) or proof of disease from Health Care Provider 
  7. Meningococcal Conjugate Vaccine (MCV4 or MenACWY; Menactra or Menveo) Grades: 7-11; 1 dose; and 12th grade, 2 doses or 1 dose if the dose was received at 16 years or older.

Pre - Kindergarten Requirements: 

  1. One dose of Varicella vaccine 
  2. One dose of MMR 
  3. Hepatitis B (HBV) 3 doses
  4. Diphtheria or Triple vaccine (DTP/DTAP) 4 doses
  5. Polio Vaccine (TOPV/IPV) 3 doses
  6. Haemophilus influenza type b (HIB), 1-4 doses before fifteen months of age or one dose on or after fifteen months of age 
  7. Pneumococcal vaccine, 1-4 doses, given at age-appropriate times and interval 
  8. Lead results

An immunization certificate must be provided. The certificate must be stamped and signed by a doctor or clinic.
