Request for IEP Translation

The Central Islip UFSD provides families with translated Individualized Education Programs (TIEPs) in any language at no cost. Eligible families must have a child with an Individualized Education Program (IEP) who is currently school-aged or is turning 5 years old in the current calendar year. If eligible, families may request a translation of the IEP at their child’s school or by selecting one of the options below. Once the translation service is completed, the District’s Special Education Department will arrange delivery to the requesting family.

You may use this form to request a translation of your child's upcoming Individualized Education Program (IEP) for services to school-age students if any of the following requirements are met:  Your child is a registered and attends a Central Islip public elementary, middle, or high school or approved and/or recommended alternate placement.; Your child will be 5 years old this year; A translation of your child's IEP will be processed after you have completed this form and when the English version of the IEP is finalized.

Once the translation request is received and processed the department will arrange for the document be translated. Once Frontline Translation is completed, the Distric IEP team will arrange delivery to the requesting family. The exact timeline for translation depends on the specific IEP.
