Central Registration, Residency & Enrollment

Please come to the Central Registration Office with all address changes.  Please bring with you (Parent/Guardian) a photo ID of yourself along with:

Owner: Picture ID and your current Mortgage Statement or current Tax Statement, and TWO current bills (water, gas, electricity, internet, cable and oil). 

Renting:  Picture ID, Landlord Affidavit attesting to residency, rental statement/agreement, and TWO current bills (water, gas, electricity, internet, cable and oil).

Universal Pre-Kindergarten (UPK) is a New York State funded program specifically designed for 4-year-olds not eligible for kindergarten. Each year, the Central Islip School District invites eligible children who reside in the district to apply for the Pre-K (UPK) Lottery. Visit the District web page in January for up-to-date information.
For kindergarten, children must be 5 years old on or before December 1 to be eligible to register.

New York State Education Law (Section 3205) states that children who turn 6 years old on or before December 1 are required to attend instruction in September of that school year.  Children who turn 6 years old after December 1 are required to attend school the following September.

Information about kindergarten registration will be available on the Central Registration webpage. 

Please use the "School Age Calculator" below to determine the year your child will be eligible to start school in September of that school year.  

You can translate documents up to 10 MB in any of these formats: .docx, .pdf, .pptx, .xlsx. PDF files must be 300 pages or less. 

Important:

• Document translation isn’t available on smaller screens or mobile.
• Images or text in images will not be translated.
• Scanned documents or photos of documents may not translate.

Enrollment Parent Support

Required documentation must be uploaded during the application process. If you are having difficulty uploading a document due to size restrictions, please save as a Reduced Size PDF (File>Save As Other>Reduced Size PDF). Please upload documents in either a PDF or JPEG format.

Students MUST be registered by a parent/guardian/or sponsor. Please provide the following:

• Proof of residency (see below) 
• Child’s up-to-date immunization records & most recent physical, with Lead results signed and stamped by physician. NYS Education request a dental certificate at the same time the Health Appraisal is required. 
• Proof of child’s age (see below)
• Proof that adult registering child is lawful custodian (see below)
• If transferring from another school, please also provide the child’s most recent report card

PROOFS OF RESIDENCY

Owner: Picture ID and your current Mortgage Statement or current Tax Statement, and TWO current bills (water, gas, electricity, internet, cable and oil). 

Renting:  Picture ID and TWO current bills (water, gas, electricity, internet, cable and oil), and an unsworn Landlord Affidavit that can be printed from our website or picked up from our office.

Any TWO (2) of the following current bills: 

• Utility Bills, including water, electric, and/or gas 
• Cablevision or satellite, oil delivery 
• Tax Bill – Homeowner’s Property 
• Income Tax form • Driver’s license
• State or other government issued identification 
• Mailed Bank Statement 
• Pay Stub (cross out Social Security number) 
• Child custody documentation, judicial custody orders or guardianship papers with the Central Islip address.
• Membership documents (library card) based on residency
