Data Privacy Agreement Form

If any website, app, or online service requires a student, staff member, or administrator to log in, create an account, or enter any personally identifiable information (PII), that platform may not be used unless a fully executed Data Privacy Agreement (DPA) is in place with the vendor. By contrast, a Data Privacy Agreement is not required for websites that are purely informational and do not require a login, account creation, or the entry of any student or staff identifying information (PII).

Examples of websites that generally do not require a DPA include informational websites where users only view content and do not log in or provide any personal information. Examples may include the district website, public museum websites, online reference pages, public news sites, weather websites, and other informational resources that are accessed without accounts or data entry.

In the clearest possible terms:

  • No login and no personally identifiable information entered = no DPA required.

  • Any login, account creation, or entry of personally identifiable information (name, id, email address, etc.) = DPA required before the site may be used.
