FOIL Application

Student Records/Transcripts are NOT subject to FOIL.  Kindly contact the building you attended for your own personal records.

The Freedom of Information Law (FOIL) allows an individual to request existing public records from the agency responsible for maintaining those records. Central Islip has a designated Records Access Officer who is responsible for accepting and responding to FOIL requests. To make a request for existing public records, please complete the form below.

When you submit this form, FOIL allows the Central Islip School District five (5) business days to make the record(s) available, deny access to the record(s), or acknowledge receipt of your request and inform you of the timeframe required to respond to your request.

Record Requests Via Electronic Mail: In accordance with Chapter 182 of the Laws of 2006, the Central Islip School District accepts requests for records submitted via electronic mail. Those records to which access is granted will be provided electronically to the extent practicable. Those documents which the Central Islip School District is not able to provide electronically will be provided via first class mail for the statutory charge of $.25 per page.

Requests for public access to records under the Freedom Of Information Law (FOIL), may be submitted electronically using the Electronic Request for Public Access to Records Form. Subsequent communication using this method will be via e-mail.

Please click on the form to Download, Print, Save, or View.
